A small handful of these presentations were given to the NASA contractor employees who worked around the clock to bring the Apollo 13 crew home safely. The certificate was gifted to "Doug Hackett", at the time of Apollo 13 he was working in the management and data systems sector of Lockheed Martin. 
 
The certificate is in mint condition and measures  4½" x 6½". The folder featuring the mission insignia has sustained some staining but is otherwise in good condition.
